HOW can i receive home health services?

What makes me eligible for home health CARE?

HOMEBOUND STATUS

You may qualify for "homebound" status if you find that leaving home to shop, to occasionally attend religious services, family functions, etc.,  requires a taxing effort or the need for an assistive device or assistance from another person.  In such circumstances you probably meet the "homebound" requirement.  Being considered "homebound" does not mean you can NEVER leave home.

COVID WAIVERS

If you have recently been diagnosed with Covid,  home health care will be postponed for at least five days after your diagnosis.  Otherwise, our staff will begin or continue home health care and will follow all required protocols to prevent further transmission of this disease.

DOCTOR'S ORDER

Home health can begin with an order from your doctor, your hospital or other medical facility on discharge to your home.  You must have an primary care physician who will follow your care with Hacienda Home Health Care.

MEDICAL NECESSITY and SKILLED NEED

Medicare certified home health agencies may provide services only when there is a "skilled need".   These "include, but are not limited to:

1.  Wound, incision, drain, g-tub , or aftercare for surgical AND non-surgical fracture repair ( e.g. joint replacements)

2.  Post hospital care for diseases such as pneumonia, sepsis, or other acute illnesses

3.  Education and training of patients and caregivers.

4.  Home exercise program for safety, strengthening and/or post surgery or  injury  care.


How much does home health care cost?

There is NO CHARGE to you for services provided by our clinical staff.

Wound Care supplies, if your primary insurance is Medicare, are covered.

 

All home health care services are paid, in full, by your insurance whether it be Medicare, Medicaid, or other. If your primary insurance is Medicare, all medical supplies are paid for by Medicare. Otherwise, most medical supplies for wound care, ostomy care, or catheter care are paid for by your insurance provider, but you may be required to pay a CO-PAY for such supplies. Hacienda Home Health Care staff helps to order these supplies and they are delivered to your home after you pay the co-pay.
